Medicaid Reform: Challenges and Opportunities for Colorado
CHI provided this fact sheet and presentation to policymakers and other health care leaders who attended the 2012 Hot Issues in Health Care legislative conference.
The Accountable Care Collaborative (ACC) plays a key role in transforming the way that Medicaid is delivered and paid for in the state of Colorado. The strategy: Create medical homes for enrollees, emphasize preventive care, and focus on overall care coordination starting with a primary care provider. The overarching goals: Reduce costs, improve health outcomes and provide a better care experience for patients and providers. Using the initial data from the Colorado Department of Health Care Policy and Financing (HCPF) on the program, we take a look at how the program has fared so far in reducing health care spending and unncecessary services.
