CHI’s Social Health Information Exchange Work in the Spotlight

CHI’s efforts to develop a social health information exchange (S-HIE) for the greater Denver metro area were profiled in Leveraging Community Information Exchanges for Equitable and Inclusive Data: CIE Community Profiles, a publication by San Diego’s Community Information Exchange.

S-HIE is an effort to build a community of care around individuals and families. An interoperable S-HIE ecosystem creates connections and strengthens coordination among health, human, and community-based service providers to offer more holistic, customized care for people in need.
